SCENARIO
A new multi-storey car park is to be erected in the centre of a large busy town. The car park will include an underground level, a ground level, and a first level. Dig & Build Construction (DBC) is a large construction company and will be the principal contractor (PC) for the job. It has previously constructed five other multi-storey car parks. There will be 50 workers on site for the duration of the build, which will take place over the warmer months to take advantage of additional daylight hours. There are also five supervisors, a land surveyor, and a site manager (SM). Before the work begins, a pre-construction phase meeting is held, and a construction phase plan is put in place.
All workers have received an induction; this covered the risks on site, along with site rules such as specific personal protective equipment (PPE) to be worn, the use of two-way radios, and making sure that all workers understand the safe system of work. The driver is concerned that, if it is found out that the cause of the incident was the vehicle tipping, they will get into trouble with their employer. So, while everyone is distracted, they wedge some pieces of wood under the outriggers so that the vehicle will not tip again.
